Did you know... about Advance Care Directives?

An Advance Care Directive (ACD) formally documents what healthcare treatments you would want or refuse if you have a life-threatening illness or injury.


Having an ACD means that others will not have to make difficult end-of-life choices for you while they are under duress.


If you are treated in, or admitted to, hospital you will likely be asked whether you have an ACD.


Two-thirds of adults in NSW do not have an ACD.
